Tavaliselt toodavad rõuged diskreetse lööbe, kus mädavillid asetsevad nahal eraldi. Lööve on kõige tihedam näol ning tihedam jäsemetel kui kerel. Enamusel juhtudel esineb ville ka peopesadel ja jalataldadel. Mõnikord villid liituvad, moodustades kokkuvalgunud lööbe, mis hakkab eraldama välimist kihti nahka selle aluseks olevalt lihalt. Patsiendid, kellel on kokkuvalgunud rõuged, jäävad sageli haigeks ka pärast seda, kui kärnad on moodustunud üle kõigi kahjustuste. Ühel vaatlusel oli suremus kokkuvalgunud rõugetesse 62%.
